Ensure the qo user has sufficient permissions to access the HSM. If using a Thales Luna HSM then this primarily means that the qo user should be in the hsmusers group. If this group existed already when the HSM Reseed software was installed, then it should have been added automatically, but if the Luna software was installed after the HSM Reseed software then the user might not be a member of the group. Run sudo usermod -aG hsmusers qo to add the user to the group.
